  • Completed application form
  • Proof of ownership/ power of attorney
  • Payment of fee
  • A “Final Reading application” must accompany the “Request for Disconnection Form.”
  • All balances on account must be cleared before the account is disconnected.
  • To carry out this task, a “Request for Disconnection form” will need to be completed and submitted at least one week before the required service date.
  • To turn off your service, we will need unassisted access to your meter.
